Are two things that waste a lot of time and energy.
Because sometimes the blame becomes as a result of lack of information or misunderstanding of something, so the other person try to explain this information, misunderstanding, or redirect the blame to another person to defend oneself against this blame. And other times even if the information is complete, it is a human temperament to justify and defend oneself.
So I think a solution could be to go to the solution directly without wasting time in blaming, and as a result in justifying. Another solution could be to make the blame less personal and to know that unintentional mistakes can happen and the right thing is to find a solution not a blame, so that the person doesn't feel the need to justify or explain.
From my experience, I have faced these situations where I wasted a lot of time and energy.
